胃癌的肿瘤标志物胃癌的肿瘤标志物（1）胃蛋白酶原I(PepsinogenI,PGI）：胃蛋白酶原是由胃粘膜分泌的胃蛋白酶前体。应用聚丙烯酰胺凝胶电沪可从人胃粘膜中分离出7种胃蛋白酶同工酶原。根据其生化性质和免疫原性不同分成二个亚群：PG1包括PGI-5；PGII，即PG6-7。PGI仅存在于胃底和胃体部粘膜的主细胞、颈粘液细胞，而PGII则分布于全胃及十二指肠的Burnner腺细胞。二者均可在血清与尿液中侧出。但由于肾小管对各种胃蛋白酶同工酶原的重吸收率不同，尿液中各胃蛋白酶同工酶原的含量与胃粘膜中的并不完会一致。因此，血液中胃蛋白酶同工酶原水平能更准确地反映胃粘膜中的情况。胃癌患者PGI的含量及PGI/PGII比值均明显降低，测定PGI水平与PGI/PGII比值可作为胃癌诊断的一个辅助指标。PGI与胃酸的分泌存在平行关系。因此，PGI水平和PGI/PGII的比值能够反映胃粘膜的功能状态，并且与胃粘膜萎缩的范围及严重程度显著相关。鉴于中、重度萎缩性胃炎与肠型胃癌的发生关系密切，PGI或PGI/PGII比值明显降低预示患肠型胃癌的危险因素增加。（2）涎酸化Tn抗原（sialylTnantigen,STn)和Thomsen-Friedenreich抗原（T抗原）：STn是一种异常糖链抗原有核心部分，其化学结构为α（2，6）-N-乙酰半乳糖胺[α（2，6）-N-acetylgavtosamine]结合了唾液酸。是Thomsen-Friedenreich抗原(T抗原)的前体，胃肠道和卵巢恶性肿瘤患者血液中STn含量升高,而且对估计肿瘤的预后有一定帮助。STn阳性胃癌的预后较差。Kakejy等研究了180例BorrmannIV型胃癌患者癌组织中STn抗原表过，其阳性率为32。STn阳性与肿瘤的范围、淋巴结转移以及腹腔播散显著相关。1/3的癌细胞STn染色阳性者其血清STn水平也升高，而生存率降低。表示肿瘤增殖活性的细胞核增殖抗原（PCNA）标记率在STn阳性组显著高于STn阴性组。但亦有报道认为STn与胃癌的分化程度的阶段无关，没有独立的预后价值。（3）对氨酰脯氨酰二肽氨基肽酶（glyclyprolinedipeptidylamminepeptidase,GPDA）：胃癌患者血清GPDA活性明显降低，与正常人及胃肠道良性病变相比差异显著。引起GPDA活性下降的机理尚不清楚。当胃癌发生肝转移时，GPDA活性常升高。因此，测定血清GPDA活性对胃癌的诊断及判断有无肝转移均具有一定意义。（4）巨分子肌酸激酶同工酶（manro-creatineKinase2,M-CK2）：M-CK2与普通肌酸激酶同工酶CK-BB，CK-MB和CK-MN不同，电沪时向阴性迁移，MW350KD。化学和激发能谱分析显示其与线粒体的CK相似，并能与抗线粒体CK的抗血清发生反应，故认为来源于线粒体。在正常人血清中M-CK2不能检出，而胃肠道癌肿患者血清中可以出现。有人测定了32例不同病期与不同组织类型胃癌病人血清中M-CK2，阳性率为75，良性胃病组仅为15.6，而正常对照组无一检出。尤其是I期和II期胃癌患者M-CK2的检出率为100，随着病变发展到III、IV期时其阳性率逐步降为61.5及33.3，提示M-CK2可能有助于胃癌有早期诊断。从组织类型来看，管状腺癌组M-CK2的阳性率明显高于印戒细胞癌、粘液腺癌和低分化腺癌组。太后动态检测亦有助于判断预后和观察疗效。（5）胃泌素(gastrin)：正常情况下，胃泌素是由胃窦部的G细胞分泌的，通过血液循环作用于细胞的胃泌素受体，使胃酸分泌增加。McGuigan等报道了胃癌患者空腹血清胃泌素浓度较正常对照组显著升高。特别是胃体胃癌其血清胃泌素水平最高，胃窦体交界部癌次之，而胃窦癌其血清胃泌素水平则与正常组无明显差别。随后的许多研究都表明了胃癌患者血清胃泌素升高，且胃底、胃体、胃窦部的血清胃泌素水平存在着显著差异，说明血清胃泌素水平主要与胃癌部位有关。
