灌肠疗法灌肠疗法【概述】灌肠法用药末加入油脂或肉汤制成药液，注入肛门灌肠，使病邪随大便排出，治疗腹部以下疾病的一种方法。在《月王药诊》中有专章叙述灌肠疗法。《四部医典》中专书两章，详细阐述灌肠法的理论依据及操作方法。另外，许多藏医名著中均介绍此法，延用至今。【疗法介绍】1、缓导泻（1）药方：分柔导泻方、洗导泻方、洗柔泻方3种。①柔导泻方：肥羊肉汤、牦牛乳、融酥油加川木香、荜茇、柏树叶、光明盐、诃子研末。用于治疗单一性隆病。②洗导泻方：用水生动物，如鱼、蛙、蝌蚪等肉汤，黄牛乳，黄牛乳中提取的融酥油，加上述川木香等五味药末，再加疣果大戟。用于隆、赤巴合并症。③洗柔泻方：用生活在干旱地区的动物肉汤，如虎豹、糜鹿、野驴、野牛等，山羊乳、山羊乳提取的融酥油，加上述川木香等五味和无患子。用于培根龙合并症。加减法：上述缓导泻方中，治疗单一性龙病者加阿魏、紫硇砂。培根龙合并症者，加干姜、荜茇。龙赤巴合并症者，加木藤寥、麻花艾花。痞瘤病加硼砂。虫病加信筒子。胃肠胀满加川木香。（2）用法：使用柔导泻方用量为半捧；洗导泻方为：捧；洗柔泻方为半捧。药液温度30℃左右。先将药液装入羊肚子或膀胱制成的灌肠器中，在口部扎一根8指长导管，尖端必须尖而光滑，涂以油脂。灌肠时，患者俯卧于床，臀部抬高，肛门涂以润滑油脂。先将灌肠器中空气排尽，然后将导管插入肛门深约3～4指左右，频频挤压灌肠器，只剩少许为止，拔出导管，用手揉搓腹部，如有泻意，尽量忍耐，待不能忍耐时，排泄数次，然后提起患者两足，摇动身体，拍击足心，促使继续泄泻。使用柔导泻方，一般在日落时灌肠，黄昏可泻出。若逾期不泻者，可再以峻导泻法灌肠。若龙势偏盛、胃火较强者，可连续施用本法数次，其它疾病，只用一次，术后用温水灌肠催泻几次即可。2．峻导泻（1）药方：分中和、锐利、温和3种。①中和方：疣果大戟、光梗丝石竹、碱花、酒曲、花斑蝥，屋梁吊挂烟灰共研细末，用牛黄溲或酒坛中剩的浊酒调汁。利器创伤，再加动物胆汁；急腹痛加短尾铁线莲；下部热症加大黄；腹胀加角蒿；虫病加白蒜；寒症加荜茇。②锐利方：即前方按病情加减后，再加狼毒。
